Why do we have to fill out this form, and why EVERY year?

  • This information is shared with the police and fire departments in case there is an after hours emergency and we need to contact someone about your business.
  • The information can be used for various municipal reporting purposes (e.g. How many of what types of businesses operate in our town?).
  • Filling out a new form each year helps us to keep all the information updated and current in our system and if any errors were made in the past (due to illegible handwriting on forms, for example) the errors can be caught when reviewing the next year’s form.

What does “Key Holder” mean?

It means: “Does the person we are calling hold a key to the property in case an emergency worker needs to get in for any reason?”

How many emergency contact names do you need?

We need as many as you wish to give us. The names will only be used if there is an emergency and we will contact in the order they are listed on the form until someone is reached.

Why do you need the alarm information? 

There may be a time we would need to call the alarm monitoring company in the event the alarm is triggered. Or if an emergency worker would have to do something that may trigger the alarm, we would want to let the alarm monitoring company know the circumstances.

Why is there a late fee?

We set deadlines to make our operation run more smoothly and when a business is late in registering, it makes our processes more difficult to maintain. Therefore, as an incentive to get the information to us in a timely manner, we charge an extra fee if the business registers after the set date due. This is a uniform fee and the State Board of Accounts, our auditing agency, will check to be sure these rules are followed.

Why are you asking for a Recycling Plan? 

This form requests information that is required by the Lake County Solid Waste Management District to comply with rules regarding recycling grants.


*This is a registration with the Town of Munster. Any licensing required for your business (e.g. state or county) is your responsibility.
